UPDATE 4PM BST: The patch is 15GB on console and between six-15GB on PC, community manager Marcin Momot has revealed. The Hearts of Stone expansion will be a separate download (comes out 13th October). But according to Momot, speaking in an email, the HOS download will be "considerably smaller".

The update is fully free for all players who already own the game on the Playstation Store. It downloads automatically to your gaming platform, provided you already own The Witcher 3. If, on the other hand, you have never had the opportunity to embark on the great adventure of CD Projekt Red, you have the option to buy the game on your console or computer, and enjoy this graphical upgrade directly. Just remember that this 4.0 patch is exclusive to next-gen consoles and PC.
The Witcher 3 HD Reworked Project is mostly compatible and for full compatibility you can download the compatibility patch, see "Installation" for details. If you do not use the compatibility patch for whatever reason, make sure to give True Fires priority. In that case you won't have HDRP meshes & textures for the square braziers and metal wall torches.
Increased Draw Distance and Increased Draw Distance for Toussaint are mostly compatible but if you enable ultra draw distance and have the GOG version of the game, make sure to download the compatibility patch, see "Installation" for details. Otherwise you might experience lights flickering at certain distances and camera angles in Novigrad (only tested with Increased Draw Distance for Toussaint though).

The PC version's patch is between 6GB and 15GB depending on your platform. Presumably the patch is so weighty because it includes content from Hearts of Stone, the expansion pack going live next week.
